Location: Conecuh County Jail- Evergreen, Alabama

Description: Adult Care Coordinator (case manager)

Salary: $35,000+ depending on experience

Qualifications: Must have a bachelor degree, drivers license and insurable driving record. Must be organized, written and verbal skills essential, and must be able to efficiently schedule and carry out appointments.  

Preferred Experience:  Bachelor degree preferrably in psychology, social work, criminal justice, health care management. Experience with individuals with mental illness. 

Duties and Responsibilities: Provide case management services to individuals currently in the Conecuh County jail with mental illness diagnosis. Provides needs assessment and completes case plan for consumers. Will work closely with master level therapist to determine consumer needs. Assist consumers with budgeting, housing needs upon release, securing Medicaid/Medicare benefits upon release as needed, medication management, making and keeping doctor/therapist appointments, and working with surrounding mental health centers as individuals are released back to their area for follow-up services. CM will provide services primarily in jail/home/community setting as well as the occasional outpatient setting when appropriate. Travel is expected around the Conecuh County area with either an agency vehicle or personal vehicle with paid mileage.
